Please call anytime to arrange an appointment. If I am unavailable to take your call personally, please leave a message. Your call will be returned as soon as possible. Office hours are by appointment only.
 

Fees:
My fee is $115 per hour.
 

Insurance:
Insurance plans often apply, but it is advisable to contact your insurance company for an authorization prior to an initial appointment, in case this is required by them. They may list me as John J. Collins or as Jack Collins. Please note, I am NOT a Medicare or Medicaid provider.
 

Services:
